The Prymorsky Regional Burn Center is one of the largest in the Far East federal district.

Annually 600-700 patients with thermal trauma receive treatment there. About half of them are children.

We offer care for citizens of Vladivostok, Primorye Territory, and also Sakhalin and Kamchatka. Our center is offering medical care not only for citizens of Russia but also from other countries. In 2006, 5 citizens of China and 2 citizens of Vietnam received treatment.

< Ãð FONT class="Apple-style-span" face="Times New Roman" size="4">Now there is a citizen of China in the center who has burns on about 70 % of the total body surface area (TBSA) , ( 45 % - full-thick- ness burns). The modern methods of treatment allow successful treatment of patients with extensive deep combustions.

This case took place in 2004. A 10 year old boy with deep thermal burns ( about 70 % TBSA), and a serious initial burn shock was delivered to our center.

In the intensive care unit the patient received adequate therapy and burn shock was stopped.

At the first day for a decompression, the necrotomy (dissection of dead tissues) was performed. Then stage by stage a necrotomy (excision of dead tissues) was performed on 10-15 of % TBSA for once. We used dressings as substitutes for skin. Then skin autografting by both intact, and perforated grafts was performed.

Because of deficiency of donor dermal resources for a transplantation we did explantations from one field 3-4 times, and from the head - 6 times. After 58 days we could completely cover all burn wounds.

The patient was released from the center for an out-patient treatment.

However, the large problem was the development of scar contractures of a neck and upper extremities, and also hypertrophic scars on different fields of a body. For the patient, we performed 6 reconstructive operations. The scar contractures and the hypertrophic cicatrixes were removed.

Now the child studies at his usual school, has good progress, and is engaged in sports.

In Prymorsky Regional Burn Center not only burn patients are treated, but also those with complications from burns.

In January 2005 a patient, citizen of Sakhalin was hospitalized in the burn center. He had severe scar deformations of the neck and thorax. Scars deformed the trachea, that caused stenosis and difficulty breathing. As the patient stenosis progressed, the difficulty of breathing increased. One year ago the patient had received full-thick burns of neck and thorax. He had received treatment in the Sakhalin regional hospital. Then during one year deformations appeared. For reconstructive operations the patient addressed to various hospitals of the Sakhalin oblast and Khabarovsk kray. He refused operative treatment. But the surgeons of these hospitals doubted if reconstructive surgery would work. Therefore they gave up on operative treatment.

The patient applied for treatment in the Prymorsky regional burn center. The patient was consulted by the surgeon-otolaringol Ãð ogist and by the thoracal surgeon. There were large doubts about reconstruction. However, the difficult operation of elimination of a cicatrical contracture of a neck, remission of a trachea from cicatrixes and its reconstruction was carried out. The postoperative period was without complications. Six months after the operation the patient felt satisfactory. At a control bronchoscopy the stenosis of a trachea was not detected. After 2 years only cicatrical contracture of a neck of a mild degree remained in the patient. But the patient already refuses subsequent reconstructive operations. The surgeons of the Prymorsky regional burn center want to increase the opportunities for the help by the patient with serious consequences of a burn trauma. They want to arrange contacts with clinics (clinic) in USA for an exchange of experience and learning.

Victor Usov graduated from the Vladivostok State Medical Institute in 1982. At that time he began to work as a surgeon in the Far East District Medical Center (Hospital) and as assistant of the professor of general surgery chair of the Vladivostok state medical institute.

He studied questions of active surgical treatment of purulent wounds and surgical infection contamination, including purulent complications of Diabetes. In 1988 he defended his dissertation as a candi Ãð date of medical sciences (American equivalent - doctor of philosophy) on a theme of treatment of these processes.

In following years, Usov Victor became the scientific director of Prymorsky regional center of burn trauma. Due to his efforts it was possible to introduce modern methods of active treatment of patients with a thermal trauma in the burn center. This has allowed improvement in the results of treatment, to reduce a lethality, to reduce treatment terms. Between 2000 and 2006 he has published 37 scientific articles and 1 monograph on treatment of patients with burn trauma. He presented reports at the All-Russia conferences of combustiologists. In 2006 Usov Victor defended the dissertation of the doctor of medical sciences (the second Russian scientific degree) on treatment of burn trauma and became the professor of general surgery and chair of the Vladivostok state medical university.

Despite successes of treatment of patients in the acute period of a burn trauma, there are unsolved problems of prophylaxis and treatment of postburn complications - scars, contractures, anatomical and cosmetic defects. Therefore Usov Victor has developed the design of the Center for rehabilitation of the patients with postburn complications.

The other part of his medical and scientific interest is treatment of purulent diseases, including purulent complicati Ãð ons of Diabetum. In Primorye Kray there is no effective medical care for patients with surgical complications of Diabetum. Therefore professor Usov has developed the design of a Center for prophylaxis of purulent complications of Diabetum, and also for effective conservative and operative treatment for these patients.

Professor Usov is an active and progressive surgeon and scientist. He is searching for contacts with doctors and scientists worldwide for successful progression of his designs.
